This form authorize Wishing on a Star Travel, LLC and its Agents to apply payment of deposit/regular payment and/or final payment to the card on file as discussed via phone and/or email. 

Fill out this form to acknowledge that you have received and read the terms and conditions provided for you from the vendor and Wishing on a Star Travel, LLC.

Only fill out this form if you are choosing to waive travel protection for your vacation.  By filling this form out, you are choosing to continue with your vacation WITHOUT travel protection.

Wishing On A Star Travel, LLC requires this form be completed by the lead guest in each party for all 'closed loop cruises' in which the guests are electing to not purchase US Passports for their travel.
